The National Heart Foundation – Victoria (NHF-V) partnered with Murray Primary Health Network (Murray PHN) to support a quality improvement project – the Heart Failure Nurse Ambassador Project.

The target audience for the project was general practitioners, practice nurses, Aboriginal Controlled Community Health Organisations (ACCHOs) and other primary health providers in the Murray PHN catchment area.

Working together with Larter consulting, HMA was engaged to

  • Consult with NHF-V (project manager and project officer), Murray PHN (Central Region and North Eastern Region), and general practices and ACCHOs initially engaged in the project.
  • Provide a thematic analysis of consultation results, using the Project Management Body of Knowledge project management knowledge areas.

The project commenced in December 2018 and was completed in January 2019.
